How your information will
be used by us and to whom we may need to disclose it
That you may request access
to the information that we hold about you

PRIVACY STATEMENT
New legislation is now in force, which protects your privacy
and covers collection and use of personal health information.
Personal health information refers to your health, medial
history or past or future medical care and how it is identified.
Any information collected
about you is considered necessary in the course of providing
comprehensive health care to you.
Your privacy is respected
and information is only accessible to authorised practice
staff.
All staff are required to
sign a 'confidentiality' clause on commencement of employment,
which prevents discussion of patient information outside
the clinic.
Personal information is
not disclosed to 3rd parties, eg: other health services,
family members, except with your consent.
If considered necessary
by your treating doctor we request your consent to provide
health information to other health service providers
eg via referral to specialists or diagnostic services.
Please let us know in advance
if you are authorising another person to collect results,
scripts etc.
If you would like to discuss our
Privacy Policy in more detail please ask at reception.
Alternatively, talk to your practitioner or the Practice
Manager.
